What Is All-on-4, Exactly?

All-on-4 is a full-arch dental implant technique. Four titanium posts are placed in the areas of the jaw with the most available bone. The two rear implants are typically angled at 30–45 degrees — a deliberate design choice that reaches denser bone deeper in the jaw and maximizes implant-to-bone contact, often without requiring bone grafts even in patients who have experienced significant bone loss.

A fixed prosthetic arch — a complete row of teeth — is then attached to those four implants. This arch stays in. It's not removable. You brush it, floss it, and use it without thinking about it. The efficiency of the design (four implants instead of one per tooth) means fewer surgical sites, faster healing, and a lower overall cost compared to a tooth-by-tooth restoration.

Who Is a Candidate?

All-on-4 is typically the right conversation for patients who:

• Have lost most or all of their teeth in one or both arches
• Are currently wearing complete dentures and want a permanent, fixed alternative
• Have experienced bone loss but want to avoid full grafting procedures
• Are in adequate general health to undergo implant surgery

It's not suited for everyone. Uncontrolled diabetes, active periodontal disease, certain medications that affect bone healing, or severe bone deficiency in specific sites may require preparatory treatment first. CBCT 3D imaging is the only honest way to know — it gives Dr. Kim a precise picture of bone volume, nerve locations, and sinus anatomy before planning a single implant.

The All-on-4 Process, Step by Step

Consultation and 3D imaging. Your first appointment covers your dental history, goals, and a full-arch CBCT scan. Dr. Kim reviews the imaging to plan implant placement with millimeter precision — no guesswork.

Extraction and implant placement. If remaining teeth need removal, that typically happens at the same appointment as implant placement, reducing overall procedure count. The four implants are placed and a temporary fixed arch is usually attached the same day. Most patients leave with teeth.

Osseointegration (3–6 months). The implants fuse with the jawbone during this phase. You'll wear the temporary arch and follow specific dietary and hygiene guidelines as healing progresses.

Final restoration. Once osseointegration is confirmed — typically verified with follow-up imaging — the temporary arch is replaced with a permanent prosthesis customized to your facial features, preferred shade, and bite.

All-on-4 vs. Traditional Dentures

Patients who make the switch from dentures consistently describe All-on-4 as a quality-of-life change that goes well beyond the cosmetic. Here's the practical difference:

All-on-4: Fixed in place — no removal. Implants stimulate the jawbone, slowing bone loss over time. Near-normal chewing force. No adhesives, no nightly soaking, no risk of movement mid-conversation. With proper care, the implants can last decades.

Traditional dentures: Removable and often require adhesive for stability. Because there's no implant stimulating the bone, the jaw gradually shrinks — meaning the denture fit changes over time, requiring relines or replacements. Chewing strength is significantly reduced, which limits diet and nutrition over the long term.

Many patients with long-term dentures also carry social anxiety around slippage — something All-on-4 eliminates entirely.

A Note on Bone Grafting

One of the most common concerns from patients who've worn dentures for years is bone loss. The good news: the angled rear implant design in All-on-4 often makes full-arch restoration possible without bone grafts, even after significant resorption.
